I have an upcoming SAP 5-50 w/NIT test in the LA Area at one of Quest's labs. I noticed that one section of the form that was provided to me had a checkmark box for "observed." Does this mean that they will likely observe me while voiding? Has anybody else's form had this box as well? Has anybody out there had a UA at any of the LA sites? I've heard of some Quest labs keeping the doors open...please share your experience.

Any input would be GREATLY appreciated!! Getting very nervous.

sure does sound like it, drink alot of water and take a complex b vitiman pill before the test as you drink the water.

Just to clarify, the box is not checked already. There are three questions in the section, each with different options for the collector to check:

-Temperature between 90 and 100
Yes, No (Enter Remark)

-Specimen Collection
Split, Single, None provided (Enter remark)

-Observed (Enter remark)

Maybe, the last box is only for the collector to check and make a remark if he/she observes something unusual?? Or to explain why they insisted on observing?

But then again, maybe it means they're supposed to watch.

Anybody out there have similar forms?

No need to be alarmed. The reason why theres that option box is that if the collector suspects tampering, the collector orders a re-submission under direct observation and checks off the option box along with explaining the reason why there was an observed collection in the 'remarks' space (ie, cold sample, odd color, etc).

As long as you don't give the collector any reason to suspect tampering, you wont be observed. In short, practice is the key to a successful substitution.
